They transform the AC to DC and regulate it to 12V and 5V. The Pin DC_5V, GND and DC_12V, GND are for the external capacitors near the transformer. AC_5V and AC_12V goes to the transformer. I don't know the correct voltage of the transformer, so i named it with the output DC voltage in the table above.
