I'm going to guess that "FLASH ATTACK" is the original game file and that "FLASH ATTACK 4" is patched to run on the 40xx series, no? Apparently you can run it that way by answering no on the "PLAY STANDARD TWO-MACHINE GAME?" question. Anyway a quick analysis shows that the game loads $027A-$2000. The first chunk at $027A looks like a cassette header indicating a file "FLASH ATTACK" $03FD-$2000 I found a good page about the game with an interview of on of the original authors that stated the game was initially released on tape. My guess is that the game will work if you load it from disk, then save the range $03FD-$2000 to tape with the exact name "FLASH ATTACK" using the monitor. Quite interestingly there's a virtual machine at $1B7E which is the actual game code. On the interview page that machine is said to be a custom forth-like language.
