If you press RUN/STOP on a 264 during power up or a reset, you will end 
up in the monitor, so it could be a keyboard problem. This feature seems 
to be meant as a replacement for the RESTORE key on the C64.

I also had that problem with a partially dead ROM once.

Do you have another 264? If yes, try the ICs from the C116 there one by one.
