Is the data on the drive important? If so, it may still be recoverable using a linux box and a SCSI card. There is a great program called "ddrescue" which will try to copy every sector, but skip the bad ones. I've used it many times to recover data. Apart from that, the only thing you need to do when setting up a new drive is set the SCSI ID to 0, and you can turn off parity, but from my disassembly of the HD-DOS, it does handle it properly. Those other jumpers should probably be left alone (I would assume WP is write protect). When you do turn on the CMD-HD with the new drive, be sure to hold down both SWAP 8 and SWAP 9 for a few seconds. This will put the drive into installation mode or else it will try to search the WHOLE drive for the DOS and this could take a very long time; the serial bus will be hung while this happens. When you have it in installation mode, you should then be able to reinstall the HD-DOS.
